Traditional Chinese Uses

Ren Shen (ginseng root) is considered the most important single herb in Chinese medicine for supplementing the body's primal Qi. It is used for collapse of Qi — acute exhaustion with cold limbs and faint pulse — and for chronic deficiency of Spleen, Lung, and Heart Qi presenting as fatigue, poor appetite, shortness of breath, and palpitations. It generates Body Fluids for wasting-thirst patterns, and calms the Spirit for anxiety and insomnia. Its profound tonic action makes it the benchmark against which all other Qi tonics are measured.

Botanical Description

Panax ginseng is a slow-growing perennial herb reaching 30-60 cm tall. The plant has a single stem bearing a whorl of 2-6 palmately compound leaves with 5 leaflets each 5-15 cm long. A single umbel of small greenish-white flowers is borne at the top of the stem, followed by red berries. The characteristic taproot (the medicinal part) is fleshy, cylindrical, often branched, with fine root hairs, pale yellow-tan in color, resembling a human figure. Wild plants take 5-7 years or more to mature for harvest; cultivated roots are typically grown for 4-6 years.

Habitat:

Cool, shaded, moist hardwood forest understory; native to northeastern China (Manchuria), Korean Peninsula, and far eastern Russia. Now almost entirely cultivated due to depletion of wild populations.

Conservation Notes:

Panax ginseng is critically endangered in the wild due to centuries of overharvesting and habitat destruction. Wild ginseng commands extremely high prices and wild populations are nearly extinct outside protected areas. The vast majority of commercial ginseng is now cultivated, primarily in China, South Korea, and North America (P. quinquefolius). CITES Appendix II listed.

Active Constituents

Ginsenosides (Rb1, Rg1, Re, Rd, Rc, Rg3)

Dammarane-type triterpenoid saponins

Concentration: approx. 2-6% of dried root (total ginsenosides)

The principal bioactive markers of ginseng. They are broadly adaptogenic, immunomodulatory and neuroprotective. Panaxadiol-type saponins such as Rb1 tend to be CNS-depressant and hypotensive, whereas panaxatriol-type saponins such as Rg1 are CNS-stimulant and mildly hypertensive, which is thought to underlie ginseng's balancing (adaptogenic) reputation. Rg3 shows the most studied anti-tumour and anti-angiogenic activity.

Ginseng polysaccharides (panaxans A-U, ginsan)

Acidic and neutral polysaccharides

Concentration: qualitative (a major fraction of the crude drug by weight)

Immunostimulant, promoting macrophage and NK-cell activity and cytokine release; the panaxan fraction contributes much of ginseng's documented hypoglycaemic activity in animal models.

Polyacetylenes (panaxynol/falcarinol, panaxydol, panaxytriol)

C17 polyacetylenic alcohols

Concentration: qualitative (minor, lipophilic fraction)

Antiplatelet and cytotoxic to cultured tumour cell lines; contribute to ginseng's inhibitory effect on platelet aggregation and thromboxane formation.

Panaxans (peptidoglycans/glycopeptides)

Glycopeptides

Concentration: qualitative (minor)

Reported to lower blood glucose in experimental models, reinforcing the polysaccharide-mediated hypoglycaemic effect.

Maltol

Pyranone (formed on steaming/red-ginseng processing)

Concentration: qualitative (trace, higher in red ginseng)

Antioxidant; a Maillard-type product generated during the steaming that converts white ginseng into red ginseng and a marker of that processing.

Gomisin-free volatile oil (sesquiterpenes: panasinsene, bicyclogermacrene)

Essential (volatile) oil

Concentration: qualitative (<0.5%)

Contributes the characteristic aroma; minor anti-inflammatory and spasmolytic activity.

⚠ Drug Interactions

Warfarin

Moderate Evidence: Probable

Panax ginseng has been reported to reduce the INR of patients stabilised on warfarin, likely through induction of warfarin metabolism and/or antagonism of its anticoagulant action. Note that this opposes the mild antiplatelet effect of the polyacetylenes, so the net clinical effect is variable.

Clinical note: Monitor INR closely if ginseng is started or stopped in a warfarin-treated patient.

Antidiabetic agents (insulin, sulfonylureas, metformin)

Moderate Evidence: Probable

Ginseng polysaccharides and ginsenosides lower blood glucose; combined with glucose-lowering drugs this can produce additive hypoglycaemia.

Clinical note: Monitor blood glucose; anticipate possible dose adjustment of antidiabetic therapy.

Monoamine oxidase inhibitors (e.g. phenelzine)

Moderate Evidence: Theoretical

Case reports describe headache, insomnia and hypomania when ginseng was combined with the MAOI phenelzine; a stimulant/monoaminergic additive effect is proposed.

Clinical note: Avoid concurrent use with MAOIs unless closely supervised.

Preparation Methods

White ginseng (sheng shai shen, sun-dried)

Parts: root

The peeled or unpeeled root is simply dried. This milder form tonifies Qi and generates fluids and is preferred when there is heat or Yin deficiency. Because ginseng is costly and its actives are heat-stable but the classic caution is to preserve potency, the sliced root is typically decocted separately (ling jian) over low heat and the strained liquid added to the finished decoction; a common dose is 3-9 g (up to 15-30 g for collapse/rescue).

Red ginseng (hong shen, steamed then dried)

Parts: root

The fresh root is steamed and then dried, turning it reddish and generating maltol and specific ginsenosides such as Rg3. Red ginseng is warmer and more strongly tonifying to Yang and Qi, favoured for cold, deficient and collapse patterns. Prepared and dosed as for white ginseng, usually decocted separately and combined.

Powder / capsule (chong fu)

Parts: root

Finely ground root taken directly by swallowing with warm water, 1-2 g once or twice daily, to conserve the expensive material and for chronic Qi tonification.

Panax ginseng in randomised controlled trials: a systematic review

Shergis JL, Zhang AL, Zhou W, Xue CC (2013) Phytotherapy Research Systematic review of randomized controlled trials

Reviewed 65 RCTs of Panax ginseng across domains including psychomotor and physical performance, glucose metabolism, respiratory disease, erectile dysfunction and quality of life. The authors concluded that many trials suggest benefit but methodological quality was often limited, so definitive efficacy claims require larger rigorous trials.

The effect of ginseng (the genus Panax) on glycemic control: a systematic review and meta-analysis of randomized controlled clinical trials

Shishtar E, Sievenpiper JL, Djedovic V, et al. (2014) PLoS ONE Systematic review and meta-analysis

Meta-analysis of 16 RCTs found that ginseng significantly improved fasting blood glucose compared with control in people with and without diabetes, though effects on HbA1c and fasting insulin did not reach significance. The overall evidence supports a modest glucose-lowering effect.

Gives a detailed monograph distinguishing genuine Shangdang and Liaodong ginseng and codifying its use for Qi collapse and wasting disorders.
