Treatment Principle

The wrist and the ankle image one another directly. Needle the corresponding point on the corresponding channel of the opposite limb.

Indications

Method

Identify the channel the pain sits on at the injured joint, find the same position on the corresponding channel at the opposite wrist or ankle, palpate for tenderness, and needle. Have the patient move the injured joint gently within a pain-free range.

⚠ Cautions

An ankle that cannot bear weight after injury, or that is markedly deformed or bruised, needs fracture assessment (the Ottawa rules are the usual guide) before any treatment.

About

The most direct of all the limb correspondences — the two joints occupy the same position in their respective limbs and carry the same channels on the same aspects.

Rationale

An acutely sprained ankle is swollen and painful to touch, which makes local needling both uncomfortable and hard to combine with movement. The opposite wrist is neither.

Clinical Notes

Most useful in the sub-acute phase, from about 48 hours after injury. In the first day or two, rest, ice and elevation do more.
