Traditional Chinese Uses

Feng Mi (honey) is a neutral, sweet substance used in Chinese medicine to supplement the middle burner, nourish Yin, moisten dryness, and relieve pain. It moistens the Lungs and intestines for dry cough and constipation, supplements Spleen and Stomach Qi for fatigue and poor appetite, and externally soothes burns and sores. As a processing medium for other herbs — particularly honey-frying licorice root or astragalus — it enhances their tonic properties. Food-grade and widely available, it is one of the most versatile kitchen medicines.

Botanical Description

Feng Mi (honey) is a viscous, sweet, supersaturated sugar solution produced by honey bees, principally Apis cerana Fabricius (the Asian or Eastern honey bee) and Apis mellifera L. (the Western honey bee), from the nectar of flowering plants. Worker bees gather nectar, concentrate it by evaporation, and add enzymes such as invertase and glucose oxidase that convert sucrose to glucose and fructose and lower pH, before sealing it in beeswax comb cells. Color, flavor, and aroma vary with the floral source. In TCM, honey is sweet and neutral, used to tonify Spleen-stomach qi, moisten dryness, harmonize the actions of other herbs (especially when honey-frying), and soothe cough.

Fructose

Monosaccharide (ketohexose)

Concentration: ~38% of fresh weight

The predominant sugar in honey, contributing to its high osmolality. The supersaturated sugar solution draws water out of bacterial cells and wound beds, producing an osmotic, desiccating antimicrobial effect and supporting the traditional moistening and nourishing actions.

Glucose

Monosaccharide (aldohexose)

Concentration: ~31% of fresh weight

Serves as substrate for the enzyme glucose oxidase, whose action generates hydrogen peroxide. Together with fructose it accounts for the high osmotic pressure and rapidly available energy underlying honey's Qi-tonifying reputation.

Glucose oxidase / hydrogen peroxide

Enzyme and derived reactive oxygen species

Concentration: Enzyme-generated H2O2 typically µmolar on dilution

Bee-derived glucose oxidase oxidises glucose to gluconic acid, slowly releasing low levels of hydrogen peroxide. This sustained peroxide generation is the principal antibacterial mechanism of most floral honeys and drives autolytic debridement in wound care.

Methylglyoxal (MGO)

Reactive 1,2-dicarbonyl compound

Concentration: Trace in most honeys; up to ~700+ mg/kg in manuka

The dominant non-peroxide antibacterial factor, most concentrated in Leptospermum (manuka) honey where it is more heat- and light-stable than hydrogen peroxide. Confers activity against a broad range of wound pathogens including resistant strains.

Bee defensin-1

Cationic antimicrobial peptide

Concentration: Variable, honey-dependent

A honeybee-secreted peptide active chiefly against Gram-positive bacteria. It contributes to the non-peroxide antibacterial spectrum and supports the traditional action of resolving toxicity (jie du).

Phenolic acids and flavonoids

Polyphenols (e.g., caffeic acid, pinocembrin, chrysin, galangin)

Concentration: ~50-500 mg gallic acid equivalents/kg

Provide antioxidant and anti-inflammatory activity, scavenging free radicals in the wound environment and modulating inflammatory mediators. Floral-source phenolics correlate with a honey's overall antioxidant capacity.

Gluconic acid

Organic acid

Concentration: Major organic acid; contributes pH ~3.2-4.5

Produced by glucose oxidase, it is the main contributor to honey's acidity. The low pH inhibits many pathogens and favours oxygen release from haemoglobin, aiding tissue repair.

Oligosaccharides (prebiotic)

Non-digestible carbohydrates

Concentration: ~4-5% of total sugars

Small amounts of oligosaccharides exert prebiotic effects that support beneficial gut flora, consistent with honey's classical roles of harmonising the Middle and moistening the intestines to unblock the bowels.

⚠ Drug Interactions

Insulin and oral antidiabetic agents

Minor Evidence: Theoretical

Honey is a concentrated source of readily absorbed fructose and glucose and raises blood glucose. In patients on glucose-lowering therapy, large or frequent intake could theoretically alter glycaemic control and complicate dose titration.

Clinical note: Diabetic patients should count honey as a carbohydrate and monitor blood glucose; ordinary culinary amounts are generally acceptable.

Preparation Methods

Direct ingestion / dissolved in warm water

Parts: Refined honey (Mel)

Take 15-30 g dissolved in warm (not boiling) water, or added at the end of a decoction. Used to moisten the Lungs for dry cough and to moisten the intestines for constipation. Avoid boiling, which degrades enzymes and delicate constituents.

Honey-frying of herbs (mi zhi / honey-processing)

Parts: Refined honey (Mel)

Diluted honey is mixed with herbs such as Radix Astragali or Herba Ephedrae and stir-fried until non-sticky. This enhances tonifying and moistening actions and moderates harshness, a standard pao zhi processing method.

Topical application

Parts: Refined or medical-grade honey (Mel)

Applied directly or on a dressing to burns, ulcers and slow-healing wounds to resolve toxicity and promote tissue repair; medical-grade sterilised honey is preferred for clinical wound care.

Honey as a topical treatment for wounds

Review of 26 trials (3011 participants) evaluating topical honey for acute and chronic wounds. Honey healed partial-thickness burns more quickly than conventional dressings and infected post-operative wounds faster than antiseptic washes, though evidence for other wound types was of lower certainty.

Honey for acute cough in children

Pooled analysis of six trials found honey probably reduces cough symptoms and frequency in children with upper respiratory infection more than no treatment or placebo, with effects comparable to some over-the-counter cough medicines. This supports honey's traditional action of moistening the Lungs and stopping cough.
