Botanical Description

Silybum marianum, the milk thistle, is a stout biennial or annual herb of the Asteraceae family native to the Mediterranean basin and now widely naturalised across temperate and subtropical regions. In its first year it forms a low rosette of large, glossy, deeply lobed leaves marked with bold milky-white veins along the midribs, the margins armed with sharp spines. In the second season a robust, branching stem rises to 1.5 metres or more, terminating in solitary purple-pink flowerheads enclosed by broad, spine-tipped involucral bracts. Flowering occurs in summer, and the heads mature into clusters of black achenes tipped with a silky white pappus. The plant favours disturbed ground, roadsides, and overgrazed pasture in full sun.

The principal active flavonolignan of milk thistle, responsible for most of the hepatoprotective, antioxidant, membrane-stabilising and antifibrotic activity; also the form used in intravenous preparations for Amanita mushroom poisoning.

Isosilybin (A and B)

Concentration: Minor component of silymarin

Contributes to the antioxidant and hepatoprotective activity of the silymarin mixture.

Silychristin

Flavonolignan

Concentration: One of the major silymarin flavonolignans

A strong radical scavenger within the complex that adds to antioxidant and cytoprotective effects.

Silydianin

Flavonolignan

Concentration: Component of silymarin

Part of the flavonolignan complex contributing to overall antioxidant activity.

Taxifolin (dihydroquercetin)

Flavanonol (flavonoid)

Concentration: Minor constituent and biosynthetic precursor of the flavonolignans

An antioxidant flavonoid that is the flavonoid partner in flavonolignan biosynthesis and adds to the free-radical scavenging of the extract.

Fixed seed oil (linoleic and oleic acids) with tocopherols and sterols

Fatty acids / lipids

Concentration: Roughly 20-30% of the seed

The seed oil carries the lipophilic silymarin and provides tocopherols and phytosterols with additional antioxidant value.

⚠ Drug Interactions

Warfarin and other CYP2C9 substrates with narrow therapeutic index

Moderate Evidence: Probable

Silymarin flavonolignans inhibit CYP2C9 in vitro at clinically achievable concentrations and can displace warfarin from albumin, which could raise free warfarin and anticoagulant effect.

Clinical note: Monitor INR when milk thistle is started or stopped in patients on warfarin or other CYP2C9-dependent drugs.

CYP3A4 and P-glycoprotein substrates

Minor Evidence: Theoretical

Silymarin inhibits CYP3A4 and P-glycoprotein in vitro, but human studies with indinavir, digoxin and other probes have generally found little or no clinically relevant change in pharmacokinetics.

Clinical note: Interaction is unlikely at usual doses; monitor if combined with sensitive narrow-index CYP3A4 substrates.

Insulin and oral hypoglycaemic agents

Minor Evidence: Probable

Silymarin has been shown in trials to improve glycaemic control in type 2 diabetes, which could add to the effect of antidiabetic drugs.

Clinical note: Monitor blood glucose when used with diabetes medication.

Preparation Methods

Standardised silymarin extract (capsules/tablets)

Parts: seeds (fruits)

Seeds are extracted and standardised to about 70-80% silymarin; typical supplemental doses are around 140 mg two to three times daily. Because silymarin is poorly water-soluble, phospholipid (phytosome) or micronised forms are used to improve absorption.

Seed decoction / tea

Parts: seeds

Crushed seeds (about 1 teaspoon) are simmered in water for 10-15 minutes and strained; a traditional liver tonic, though the low water solubility of silymarin makes teas far less potent than standardised extracts.

Intravenous silibinin (clinical use only)

Parts: seeds (purified silibinin)

A standardised silibinin dihydrogen-disuccinate preparation is administered intravenously in hospital as an antidote for Amanita phalloides (death-cap) mushroom poisoning. This is a prescription hospital treatment, not a self-care preparation.

Young leaves and stalks as food

After removing the spines, young leaves, stalks and flower receptacles have historically been eaten as a vegetable, similar to artichoke, giving the plant its common name variegated artichoke.

A Randomized Trial of Silymarin for the Treatment of Nonalcoholic Steatohepatitis

Wah Kheong C, Nik Mustapha NR, Mahadeva S (2017) Clinical Gastroenterology and Hepatology Randomized, double-blind, placebo-controlled trial

In biopsy-proven non-alcoholic steatohepatitis, 48 weeks of silymarin 700 mg three times daily reduced fibrosis scores versus placebo without significant adverse effects, although the primary histological endpoint of NAS reduction was not met.
