Botanical Description
Piper umbellatum (syn. Pothomorphe umbellata), known as cordoncillo, caapeba or santa-maria leaf, is an aromatic perennial shrub or subshrub in the family Piperaceae reaching 1 to 3 metres in height, native to tropical America from southern Mexico through Central America to Brazil and the Caribbean, and also naturalised in tropical Africa and Asia. The stems are jointed, soft-woody, swollen at the nodes and often reddish. The very large, heart-shaped to nearly orbicular leaves are 15 to 35 centimetres across, with palmate venation, a deeply cordate base and a pointed apex; the upper surface is dark green and the lower surface paler and finely pubescent. Tiny apetalous, bisexual flowers are crowded on slender white to cream cylindrical spikes 4 to 10 centimetres long, arranged in characteristic terminal umbel-like clusters of 4 to 12 spikes, which gives the species its epithet. Fruits are small one-seeded drupes embedded in the rachis. The plant emits a peppery, somewhat balsamic odour when crushed.

The signature bioactive of the roots. A potent lipid-peroxidation and peroxyl-radical scavenger reported to exceed alpha-tocopherol in some antioxidant assays. It underlies the documented topical photoprotective (anti-UVB) effect, inhibits Bothrops myotoxic phospholipase A2, and shows antischistosomal, antimalarial and cytotoxic (anti-tumour) activity in vitro and in animal models.

Preparation Methods
Leaf infusion / decoction

Traditional Neotropical use: fresh or dried leaves are infused or briefly decocted and taken for inflammatory, hepatic and digestive complaints, or applied as a warm wash. Aromatic and generally well tolerated in folk practice; avoid prolonged high-dose use in pregnancy given limited safety data.
Root extract / tincture

The 4-nerolidylcatechol-rich roots are extracted with ethanol/hydroethanol for antimalarial and antiparasitic folk use and as the basis of standardised photoprotective preparations.
Topical leaf poultice / root-extract gel
Parts: leaf, root
Crushed leaves are applied to wounds, snakebite and skin inflammation; standardised root-extract gels (about 0.1% 4-nerolidylcatechol) have been formulated for topical antioxidant/anti-UVB use. 4-Nerolidylcatechol is photolabile, so such preparations require stabilisation.
Clinical Studies
Piper umbellatum L. (Piperaceae): Phytochemical profiles of the hydroethanolic leaf extract and intestinal anti-inflammatory mechanisms on 2,4,6-trinitrobenzene sulfonic acid induced ulcerative colitis in rats
Oral pretreatment with the hydroethanolic leaf extract (30-300 mg/kg) markedly reduced colonic inflammation in TNBS colitis, lowering TNF-alpha and IL-1beta and macroscopic damage scores, comparable to mesalazine. GC-MS/LC-MS characterised the phenolic and flavonoid profile.
Inhibitory effects of Piper umbellatum and Piper peltatum extracts towards myotoxic phospholipases A2 from Bothrops snake venoms: isolation of 4-nerolidylcatechol as active principle
Extracts of P. umbellatum and P. peltatum inhibited the catalytic and myotoxic activity of Asp49 and Lys49 phospholipases A2 from Bothrops venoms. Bioassay-guided fractionation isolated 4-nerolidylcatechol as the active principle, which fully inhibited myotoxin I enzymatic activity at a 10:1 inhibitor:toxin ratio.
