Traditional Uses
Matico is a major medicinal plant of indigenous Amazonian and Andean pharmacopoeias. Throughout Peru, Bolivia, Ecuador and Colombia the aromatic leaves are applied directly to wounds, ulcers and skin infections as an antiseptic, haemostatic and vulnerary dressing; leaf decoctions and infusions are taken internally for haemorrhage, gastric ulcers, urinary tract infections and respiratory complaints (PFAF, 2023). The Spanish name 'matico' (soldier's herb) is said to derive from a wounded Spanish soldier who learned of the leaf's blood-stanching properties from indigenous healers.

Botanical Description
Piper aduncum, commonly called spiked pepper, matico or higuillo de hoja menuda, is an evergreen shrub or small tree in the Piperaceae family native to the tropical lowlands and Andean foothills of Central and South America, where it occurs from sea level to about 3,000 metres in inter-Andean valleys and along forest margins. Plants typically grow 3 to 7 metres tall, with swollen jointed nodes, alternate, lance-shaped, prominently veined leaves 12 to 20 centimetres long that are softly hairy and rough to the touch, and a strong aromatic pepper-like fragrance when crushed. The minute apetalous flowers are densely packed onto erect to recurved, slender, white to pale yellow-green spikes 6 to 15 centimetres long borne opposite the leaves, ripening into tiny dry fruits. Widely planted and now naturalised across tropical Asia, the Pacific (Melanesia, Polynesia, Hawaii) and parts of Florida, it is regarded as one of the world's most invasive woody pioneers in disturbed tropical forests.

The most frequently reported and characteristic constituent of Piper aduncum oil. Dillapiole has documented insecticidal, acaricidal, antifungal, antiparasitic and anti-inflammatory activity, and acts as an insecticide synergist by inhibiting insect (and mammalian) cytochrome-P450 detoxification. As a methylenedioxyphenyl phenylpropanoid (related to myristicin/safrole) it carries theoretical hepatotoxicity/genotoxicity concerns at high oral doses.

⚠ Drug Interactions
Drugs metabolised by cytochrome P450 (CYP)
Dillapiole, the principal constituent, is a well-characterised methylenedioxyphenyl P450 inhibitor and insecticide synergist. By the same mechanism it could theoretically slow the metabolism of co-administered CYP-substrate drugs, raising their plasma levels. Demonstrated in insect and enzyme models rather than in humans.
Clinical note: Avoid concentrated oil internally with narrow-therapeutic-index CYP-substrate medications.
Preparation Methods
Poultice / topical (fresh leaves)

Traditionally the fresh leaves are macerated, chewed or heated and applied directly to cuts, wounds, ulcers, boils and insect bites as a hemostatic and antiseptic dressing ('soldier's herb', matico). Powdered dried leaf is also sprinkled on wounds.
Leaf infusion / decoction

A tea of the leaves is used in Amazonian and Andean folk medicine as a wound wash and internally for diarrhoea, stomach pain, urinary complaints and menstrual cramps. Because the oil is rich in the methylenedioxyphenyl phenylpropanoid dillapiole, concentrated oil or high internal doses should be avoided, especially in pregnancy.
Essential oil

Steam-distilled leaf oil (frequently dillapiole-rich) is studied chiefly as a botanical insecticide/acaricide and antiparasitic agent for external and agricultural use rather than internal medicine.
Clinical Studies
Essential Oil from Piper aduncum: Chemical Analysis, Antimicrobial Assessment, and Literature Review
Ninety compounds were identified, dominated by piperitone (23.7%), camphor (17.1%) and viridiflorol (14.5%). The oil showed little antibacterial/antifungal activity but marked antiprotozoal potency (Plasmodium falciparum IC50 1.3 µg/mL; Trypanosoma brucei 2.0 µg/mL), alongside notable cytotoxicity (MRC-5 IC50 5.1 µg/mL).
Piper aduncum essential oil: a promising insecticide, acaricide and antiparasitic. A review
Critical review of 59 studies concluding that oil composition is chemotype-dependent, with dillapiole the most cited component followed by myristicin, 1,8-cineole and β-ocimene. Insecticidal effects were strong against Aedes aegypti (LC50 57-200 µg/mL) and acaricidal activity notable against Tetranychus urticae (LC50 5.83-7.17 µg/mL).
