Traditional Chinese Uses

Ku Ding Cha (bitter holly leaf tea, ligustrum or ilex leaf) is intensely bitter and cold, used in Chinese medicine to clear Heat from the head and upper body, settle Liver Yang and Fire, and dispel Wind-Heat. It addresses headaches, red eyes, sore throat, and hypertension associated with Liver Heat or Liver Yang rising. Modern research supports its cardiovascular benefits, including blood pressure-lowering and lipid-modifying effects. Despite its medicinal power, it is commonly consumed as a daily beverage tea.

Botanical Description

Ilex kudingcha C.J.Tseng and the related Ilex latifolia Thunb. (Aquifoliaceae) are evergreen trees native to southern China, growing 8-20 m tall with smooth grey bark and stout twigs. The thick, leathery leaves are alternate, broadly elliptic to oblong, 10-25 cm long, with sharply serrate margins and a glossy dark-green upper surface. Small, dull-yellow, four-merous flowers cluster in axillary cymes and develop into red, globose drupes about 1 cm across. The species inhabits subtropical montane broadleaf forests in Yunnan, Guangxi, Sichuan, and Hainan. The large mature leaves are processed by rolling and drying to produce Ku Ding Cha (bitter nail tea), a markedly bitter, cooling beverage used in TCM and folk medicine to clear heat, dispel summer-heat, and brighten the head and eyes.

Bitter triterpene saponins responsible for much of the tea's bitterness; show cholesterol-acyltransferase (ACAT) inhibition and hypolipidemic activity in vitro, and contribute to the heat-clearing, lipid-reducing reputation.

Oral hypoglycemic agents / insulin

Moderate Evidence: Theoretical

Kudingcha extracts lower blood glucose in preclinical and some clinical settings; combined with antidiabetic drugs this could produce additive glucose lowering and hypoglycemia.

Clinical note: Monitor blood glucose in diabetic patients and adjust antidiabetic dosing if needed.

Antihypertensive and lipid-lowering drugs

Minor Evidence: Theoretical

Meta-analysis data show Kudingcha lowers total and LDL cholesterol and it has mild blood-pressure-lowering effects; concurrent use with statins or antihypertensives may be mildly additive.

Clinical note: Generally additive and benign, but monitor lipids and blood pressure when combining with pharmacotherapy.

Preparation Methods

Infusion (herbal tea)

One or a few tightly rolled leaf 'nails' (about 1-3 g) are steeped in hot water and drunk as a bitter tea to clear heat, brighten the eyes and relieve thirst. The infusion can be re-steeped several times.

Water decoction

Parts: Leaf

In formal herbal use, 3-9 g of leaf is decocted to disperse wind-heat, clear the head and resolve phlegm. Because it is markedly cold and bitter, prolonged or high-dose use is avoided in those with Spleen/Stomach cold or deficiency.
