Traditional Chinese Uses
Feng Jiao (bee venom, mel apitherapy) is a warm substance used in Chinese medicine for joint and muscle pain from Wind-Cold-Damp bi syndrome, particularly rheumatoid arthritis-type conditions. Applied via bee stings or as processed preparations, it warms the channels, disperses cold and damp obstructions, and stimulates circulation. Its potent anti-inflammatory mechanisms have attracted substantial modern research interest, and apitherapy has a long history in both Chinese and other traditional medical systems for pain and inflammatory conditions.
Botanical Description
Feng Jiao is propolis, a resinous mixture produced by honey bees (Apis mellifera) and related Apis species rather than a plant tissue. Worker bees collect resin and gum exudates from buds and wounds of various trees (notably poplars, conifers, and birches), mix them in the hive with beeswax and salivary enzymes, and use the resulting sticky brown to greenish material to seal cracks, smooth surfaces, and varnish brood cells. Composition varies by geography and floral source but typically includes 50% plant resins, 30% beeswax, 10% essential oils, 5% pollen, and 5% other organic constituents, with characteristic flavonoids, phenolic acids, and aromatic esters.

Caffeic acid phenethyl ester (CAPE)

A widely studied anti-inflammatory and cytoprotective compound that inhibits NF-kB activation; investigated for anticancer, antioxidant and antimicrobial actions.

⚠ Drug Interactions
Warfarin and other anticoagulants
Propolis has demonstrated antiplatelet/antithrombotic activity and, in experimental and case data, has increased the anticoagulant (INR) effect of warfarin; possible additive effect plus modulation of warfarin-metabolising cytochrome P450 enzymes.
Clinical note: Monitor INR closely if propolis is combined with warfarin or other anticoagulants; counsel patients on bleeding signs.
Antiplatelet agents / NSAIDs (aspirin, clopidogrel, ibuprofen)
Propolis flavonoids can inhibit platelet aggregation, theoretically adding to the antiplatelet effect of these drugs.
Clinical note: Use caution and watch for bruising or bleeding when combined with antiplatelet drugs.
CYP-metabolised drugs (e.g. CYP2C9, CYP3A4, CYP2D6 substrates)
Propolis constituents (flavonoids, CAPE) inhibit several cytochrome P450 isoenzymes in vitro, which could raise plasma levels of narrow-therapeutic-index drugs metabolised by these enzymes.
Clinical note: Consider potential enzyme inhibition with high-dose propolis extracts; caution with narrow-therapeutic-index drugs.
Antidiabetic drugs (insulin, sulfonylureas, metformin)
Clinical trials show propolis reduces fasting glucose, HbA1c and insulin resistance, which may add to the effect of glucose-lowering medication.
Clinical note: Monitor blood glucose; dose adjustment of hypoglycaemic drugs may be needed.

Preparation Methods
Ethanolic tincture
Parts: Raw propolis resin (bee glue)
Raw propolis is macerated in high-proof ethanol (commonly 70-96%) to yield an ethanolic extract; taken in drops diluted in water or applied topically. Ethanol efficiently extracts the flavonoid and phenolic fraction.
Oral capsule / powder
Parts: Purified propolis extract
Standardised propolis extract in capsules or powder; clinical trials have used roughly 500-1500 mg/day. Take with food.
Topical ointment / oral rinse
Parts: Propolis resin extract
Propolis extract incorporated into ointments, gels or mouthwashes and applied to skin lesions, wounds or oral mucosa for its antimicrobial and tissue-healing actions.
Clinical Studies
The Effect of Iranian Propolis on Glucose Metabolism, Lipid Profile, Insulin Resistance, Renal Function and Inflammatory Biomarkers in Patients with Type 2 Diabetes Mellitus: A Randomized Double-Blind Clinical Trial
94 patients with type 2 diabetes received 1000 mg/day Iranian propolis or placebo for 90 days. The propolis group showed significant reductions in HbA1c, 2-hour postprandial glucose, fasting insulin and HOMA-IR insulin resistance versus placebo.
Propolis supplementation improves glycemic and antioxidant status in patients with type 2 diabetes: A randomized, double-blind, placebo-controlled study
Type 2 diabetic patients supplemented with propolis showed significant improvement in fasting blood glucose, HbA1c and antioxidant status compared with placebo over the study period, supporting an adjunct glycaemic benefit.
