Traditional Uses

Boneset was one of the most widely employed indigenous febrifuges of eastern North America. The Iroquois used infusions and decoctions of the plant for fevers and chills, as a general tonic, as an emetic, for venereal complaints, and to treat colds (Herrick, 1977). The Cherokee similarly took an infusion of the leaves as a febrifuge and for colds, as a kidney aid, and for snakebite (Hamel and Chiltoskey, 1975). The Mohegan considered it one of their most important fever and cold remedies (Tantaquidgeon, 1972). It was widely adopted into nineteenth-century Eclectic American practice as a classical diaphoretic, tonic, and bitter (King's American Dispensatory, 1898).

Botanical Description

Eupatorium perfoliatum is a stout, erect perennial herb of the Asteraceae family, typically 70 to 150 centimetres tall, growing from a knotty rhizome with fibrous roots and producing one or several densely hairy, often purple-spotted stems. The most distinctive feature lies in the leaves, which are opposite, lanceolate to oblong-lanceolate, 8 to 20 centimetres long and 2 to 5 centimetres wide, with finely serrate margins, a long-acuminate apex, a coarsely wrinkled and conspicuously veined upper surface, and bases that are fused around the stem so that paired leaves form a single perfoliate disc through which the stem appears to grow. The terminal, flat-topped, dense corymbs carry numerous small heads, each 4 to 7 millimetres across, of dull white tubular disc florets only (no ray florets), subtended by overlapping pubescent phyllaries. The fruit is a slender, black, five-angled achene 1.5 to 2 millimetres long bearing a tuft of fine bristly white pappus. Native to moist meadows, marshes, low fields, swamps and damp ditches across eastern and central North America.

Active Constituents

Eupafolin (6-methoxyluteolin / nepetin)

Flavone (methoxylated flavonoid)

Concentration: Minor flavonoid aglycone of the aerial parts

A principal anti-inflammatory constituent. In LPS-stimulated macrophages eupafolin inhibited inducible nitric oxide synthase (iNOS) with an IC50 near 6 µM and down-regulated inflammation-related cytokines and chemokines, providing a mechanistic basis for the herb's traditional use in feverish, inflammatory conditions.

Sesquiterpene lactones (euperfolide, euperfolitin, eufoliatorin, eucannabinolide, dimeric guaianolides)

Sesquiterpene lactones (germacranolide/guaianolide type)

Concentration: Bitter principles of the herb; low percentage

Contribute the characteristic bitterness and are considered immunomodulatory and anti-inflammatory. A dimeric guaianolide isolated from the herb inhibited nitric oxide production (IC50 ~16 µM) in activated macrophages.

Caffeoylquinic acids (chlorogenic acid, 3,5- and 4,5-dicaffeoylquinic acids, caffeic acid)

Hydroxycinnamic acid derivatives (caffeic acid esters)

Concentration: Major polyphenolic fraction

The dominant polyphenols of the hydroalcoholic extract. They inhibit attachment of influenza A virus to host cells in vitro, supporting the traditional use of boneset against colds and influenza-like illness.

Flavonoid glycosides (quercetin, kaempferol, astragalin, hyperoside, rutin, eupatorin)

Flavonol and flavone glycosides

Antioxidant and mild anti-inflammatory constituents typical of Asteraceae, contributing to the overall polyphenolic activity of the extract.

Immunostimulating polysaccharides (4-O-methylglucuronoxylans, acidic heteroglycans)

Polysaccharides (xylose/glucuronic-acid containing)

Concentration: Water-soluble macromolecular fraction

In vitro these polysaccharides stimulate phagocytosis and immune-cell activity, a proposed basis for the non-specific immune support attributed to boneset in minor viral infections.

Dehydropyrrolizidine alkaloids (lycopsamine, intermedine, their N-oxides and acetyl derivatives)

Unsaturated pyrrolizidine alkaloids

Concentration: Trace: ~0.0002-0.07% w/w (variable between collections)

A safety-relevant trace constituent. Unsaturated (1,2-dehydro) pyrrolizidine alkaloids are potentially hepatotoxic and genotoxic; their documented presence is the main reason prolonged internal use of boneset is discouraged, even though overt human hepatotoxicity from boneset itself has not been demonstrated.

⚠ Drug Interactions

Hepatotoxic drugs and alcohol (e.g. methotrexate, isoniazid, high-dose acetaminophen)

Moderate Evidence: Theoretical

Boneset contains trace unsaturated pyrrolizidine alkaloids that are metabolised to reactive pyrrolic esters in the liver. Concurrent exposure to other hepatotoxins could theoretically add to hepatic burden.

Clinical note: Avoid prolonged internal use of boneset in patients taking hepatotoxic drugs or drinking alcohol; avoid entirely in pre-existing liver disease and in pregnancy/lactation.

Immunosuppressant therapy (e.g. ciclosporin, tacrolimus, azathioprine)

Minor Evidence: Theoretical

The immunostimulating polysaccharide and sesquiterpene lactone fractions could theoretically oppose the intended action of immunosuppressive regimens.

Clinical note: Theoretical only; advise transplant and autoimmune patients on immunosuppressants to avoid immunostimulant herbs unless supervised.

Preparation Methods

Hot infusion (traditional diaphoretic tea)

Parts: Dried flowering aerial parts (leaves and flowering tops)

Traditionally about 1-2 g of the dried flowering herb is infused in a cup of just-boiled water for 10-15 minutes and taken hot to promote sweating in feverish colds and influenza. Taken cold it acts more as a bitter tonic. Because the herb contains trace hepatotoxic pyrrolizidine alkaloids, internal use should be short-term only (not more than a few days), and it must be avoided in pregnancy, breastfeeding, in children, and in anyone with liver disease.

Tincture

Parts: Dried aerial parts

A hydroethanolic tincture (roughly 1:5 in ~45% alcohol) is used in small doses by Western herbalists as a diaphoretic and immune stimulant. The same pyrrolizidine-alkaloid cautions apply: reserve for acute short-term use and avoid chronic dosing.

Eastern North American Indigenous materia medica (Iroquois, Cherokee, Menominee)

Native American peoples used infusions of the plant for fevers, colds and body aches; the common name 'boneset' and the epithet 'feverwort' reflect its reputation for the deep bone-pains of febrile illness (later associated with dengue, 'break-bone fever').
