Botanical Description

Cynara cardunculus, the cardoon (which includes the cultivated globe artichoke), is a robust, thistle-like perennial herb of the daisy family growing 1 to 2 m tall from a stout taproot. It forms large rosettes of deeply pinnately divided, greyish-green leaves that are often spiny, woolly-white beneath, and may exceed half a metre in length; the fleshy leaf-stalks and midribs are blanched and eaten in the cardoon form. Tall, branched, grooved flowering stems bear very large globular flower heads enclosed in fleshy, spine-tipped bracts; the artichoke 'choke' and fleshy receptacle are the edible parts of the artichoke form. The numerous florets are bright violet-blue and tubular. Native to the Mediterranean region, it grows on dry, sunny slopes and is cultivated both as a vegetable and as a bitter medicinal herb.

The signature compound of the plant, associated with choleretic (bile-stimulating), hepatoprotective and lipid-lowering actions; part of the caffeoylquinic acid complex responsible for much of the biological activity.

Chlorogenic acid

Caffeoylquinic (phenolic) acid

Concentration: Abundant phenolic acid of leaf and head

Antioxidant phenolic acid with reported effects on lipid metabolism, glucose handling and hepatic fat accumulation.

Luteolin and luteolin-7-O-glucoside (cynaroside)

Flavone (flavonoid)

Concentration: Major flavonoids of the leaf

Antioxidant and anti-inflammatory flavones; luteolin can inhibit cholesterol biosynthesis, contributing to the hypocholesterolaemic activity of leaf extracts.

Cynaropicrin

Sesquiterpene lactone (guaianolide)

Concentration: Principal bitter principle of the leaf

Responsible for the characteristic bitterness that stimulates bile flow and digestion; also shows anti-inflammatory and cytotoxic activity in laboratory studies.

Caffeic acid

Phenolic acid

Concentration: Building block of the caffeoylquinic acids, present free and esterified

Antioxidant phenolic acid contributing to the overall free-radical-scavenging and hepatoprotective effects.

Inulin

Fructan (polysaccharide)

Concentration: Major storage carbohydrate of the root and head

A prebiotic dietary fibre that supports gut microbiota and contributes to the food and digestive uses of cardoon and artichoke.

⚠ Drug Interactions

Lipid-lowering drugs (statins, fibrates)

Minor Evidence: Theoretical

Artichoke leaf extract modestly lowers total and LDL cholesterol; combined with statins or fibrates the effect could be additive, generally a benign or beneficial interaction.

Clinical note: No adverse interaction is documented; the combination is usually harmless but lipid monitoring is prudent.

Anticoagulant / antiplatelet drugs

Minor Evidence: Theoretical

The high content of caffeoylquinic acids and flavonoids has weak antiplatelet potential in vitro; a clinically relevant interaction has not been demonstrated.

Clinical note: No clinical cases reported; routine caution only.

Preparation Methods

Standardised leaf extract (capsule/tablet)

Parts: Leaf

Dry aqueous or hydroalcoholic leaf extracts standardised to caffeoylquinic acids (as chlorogenic acid) or cynarin are taken in divided doses (commonly 300-640 mg two to three times daily) for dyspepsia and mild hyperlipidaemia. Avoid in bile duct obstruction or gallstones and in known Asteraceae allergy.

Bitter leaf infusion / tincture

An infusion or ethanolic tincture of the bitter leaf is taken shortly before meals as a digestive bitter and mild choleretic. The intense bitterness is part of the intended effect.

Culinary vegetable (heads, blanched stalks)

Parts: Flower head (artichoke), Leaf stalk (cardoon)

The immature flower heads (globe artichoke) and blanched leaf stalks (cardoon) are cooked as vegetables. They are a source of inulin and phenolics but contain far lower concentrations of active constituents than standardised leaf extracts.

Clinical Studies

Artichoke leaf extract (Cynara scolymus) reduces plasma cholesterol in otherwise healthy hypercholesterolemic adults: a randomized, double blind placebo controlled trial

Twelve weeks of standardised artichoke leaf extract (1280 mg/day) produced a small but statistically significant fall in plasma total cholesterol relative to placebo in otherwise healthy adults with mild-to-moderate hypercholesterolaemia.

Artichoke leaf extract for treating hypercholesterolaemia (Cochrane systematic review)

Wider B, Pittler MH, Thompson-Coon J, Ernst E (2013) Cochrane Database of Systematic Reviews Systematic review / meta-analysis of randomised controlled trials

Pooling the available randomised trials, the review found that artichoke leaf extract can produce modest reductions in total cholesterol, while noting the small number and modest quality of trials and calling for larger, more rigorous studies.

Efficacy of artichoke dry extract in patients with hyperlipoproteinemia

Six weeks of 1800 mg/day standardised artichoke leaf extract significantly reduced total and LDL cholesterol compared with placebo in patients with hyperlipoproteinaemia, with reductions of roughly 18% and 23% respectively in the active group.

Historical Texts

Dioscorides, De Materia Medica

1st century CE
Greek and Roman writers including Dioscorides and Pliny described the cultivated cardoon/artichoke (kaktos, carduus) as a food and a remedy, including use of the root and leaf and a belief that it aided the digestion and sweetened the breath and urine.

Pliny the Elder, Naturalis Historia

1st century CE
Records the carduus as a prized, costly cultivated vegetable of the Romans, cultivated in gardens around Carthage and Cordoba.

European Renaissance herbals (e.g. Gerard's Herball)

16th-17th century
Herbalists described the artichoke as strengthening the stomach and as a liver and digestive remedy, foreshadowing the modern use of the leaf as a choleretic bitter.
